How to report a player?

How to Write a report?;
1.Make a new topic in 'Report players' (use the player (that abused) hes name in topic)
2.You need a name of the player.
3.You need a reason.
Like this:
Title of the topic 'test' (watch it if you want)
Name : test
Reason : Scamming
